Every one of these start the same way, a staff member who looked fine on paper but under
pressure reacts in a way nobody saw coming.
A wrong decision in the moment can turn a routine incident into unnecessary
injury, legal exposure, investigations, and another recruitment cycle to replace
someone who wasn't actually fit for the job.
